- By
submitting this request for your motor vehicle
record, you authorize TR Communications,
Inc to obtain a copy of your driver's license
history.
- You
certify that any information you provide
to us, is your own. If the information is
not you own or for your employee a signed
release form, by the subject must be fax
to us. See confirmation page for details
- You
understand that TR Communications, inc receives
the driving record directly from the Dept.
of Highway Safety, therefore the underlying
information received is that which is currently
available from DMV subject to availability.
- You
certify that you understand that anyone
who knowingly and willingly obtains information
on a consumer from a consumer reporting
agency under false pretenses (illegally)
may be fined up to $5,000 or imprisoned
up to one year under Title 18, United States
Code Indemnification
- You
agree to release and indemnify TR Communications.
Inc. as well as their officers, employees,
shareholders, and affiliates from all liability
arising from the unauthorized access or
improper use of the information reports
and services which are provided on this
website and pursuant to this agreement.
- You
agree to release and indemnify the credit
bureaus and all other information suppliers
used by TR Communications. Inc to provide
consumer credit information and any other
information, as well as their parent, sister,
and affiliate entities, their officers,
employees, contractors, and agents from
all liability arising from the unauthorized
access, or improper use of the information
reports and services which are provided
on this website and pursuant to this agreement
- You
certify that all information provided by
you in this agreement shall be true and
accurate to the best of your knowledge and
that you have read, understood and agreed
to the terms of this legally binding agreement.
Important: If you require a
certified copy of your driving record, you
must contact your local DMV or your local
Court House directly. We cannot provide
you with certified copies
In order to comply with Federal, State,
and Local regulations and acts, personal
information such as social security will
not be displayed in your results. Your First
Name, Middle Initial, Last Name, Address,
and Date Of Birth will only be shown if
the information provided matches the information
on the Driving Record. In addition, violations
which were dismissed, or not charged will
not be displaye

READ CAREFULLY
BEFORE SUBMITTING: This agreement is made subject to
the following terms, conditions & understandings: TR Communications,
Inc will make reasonable efforts to locate the requested
information and will make reasonable efforts to check the
accuracy of any information provided. Since TR Communications,
Inc does not prepare the underlying records its retrieves,
it cannot guarantee the accuracy, timeliness or completeness
of the information provided. TR Communications, Inc only
guarantees that it will furnish all the information it has
retrieved. TR Communications, Inc shall not be liable for
a ny loss or injury resulting from the use of, failure to
provide, or delay in receiving, such data. In the event
that no information can be obtained on the subject matter(s)
through our agreed efforts, CLIENT shall nevertheless be
responsible for the
payment of all fees agreed to be paid as specified above,
the client may be charged a dispute charge
if client disputes a charge, for which the client previously
charged as VALID and TRUE.
GENERAL USE OF DATA: The CLIENT (the person requesting
the driving record) agrees that the information supplied
by TR Communications, Inc strictly for its own use, and
that this information will NOT be used by CLIENT for any
purposes prohibited by any governmental laws, statutes,
ordinances, rules and/or regulations or with the intent
to stalk, harm, slander or hurt another in anyway.
COMPLIANCE WITH THE FAIR CREDIT REPORTING ACT: CLIENT
agrees that any requested reports or information which maybe
covered by the FAIR CREDIT REPORTING ACT must be used in
full compliance with the terms and intentions of that ACT.
CLIENT recognizes and understands that the FAIR CREDIT REPORTING
ACT provides that anyone "who knowingly and willfully obtains
information on a consumer from a consumer reporting agency
under false pretenses shall be fined not more that $5,000
or imprisoned not more than one year, or both."
DRIVERS PRIVACY PROTECTION ACT (DPPA): Described
in the DPPA Title 18 U.S.C. § 2721 et. seq. You are ordering
this driving record on yourself. Personal Information will
be provided on your driving record as described in DPPA
18 U.S.C. § 2725 et. seq. Driver's
Privacy Protection Act
ANY FACSIMILE OR ELECTRONIC SUBMISSION of this agreement
shall be deemed to be and shall serve as if it were the
original agreement between the parties, providing all legal
rights and remedies under the governing entities or other
applicable governmental authorities. Client also agrees
to pay any attorney's fees and court costs which we may
incur in collecting any unpaid amount and agrees to pay
any attorneys fees and court costs incurred by TR Communications,
Inc in defending any lawsuit brought by client or client's
representatives. Client agrees to give up right to local
jurisdiction and understands and consents to any case arising
from non-payment of a bill to be heard under Florida jurisdiction.
CERTIFICATION: By choosing to submit this form electronically,
I certify and agree that by entering/typing my name in the
space below, I bind and legally obligate myself to the same
extent as, if I would have signed my name on a printed version
of this form. I also certify that I have read, understand
and accept the Terms and Conditions and General Use of Data
stated below. I also give permission to TR Communications,
Inc to use my credit card for payment of the above requested
service.
